Invisalign Treatment Tiers: Express, Lite, Moderate & Comprehensive
The Four Invisalign Tiers
Invisalign classifies treatment into product tiers based on the number of aligners required and the complexity of the tooth movements involved. Your orthodontist or dentist determines your tier after a 3D digital scan (ClinCheck).
| Tier | Aligner Count | Treatment Duration | Cost Range (2026) |
|---|---|---|---|
| Express | 5–10 | 3–6 months | $1,800–$3,500 |
| Lite | 14–20 | 6–12 months | $3,000–$5,000 |
| Moderate | 21–35 | 12–18 months | $4,500–$6,500 |
| Comprehensive | 36+ (unlimited refinements) | 18–24+ months | $5,500–$8,500 |
Use the Invisalign Cost Calculator to estimate your after-insurance cost and monthly payments at each tier.
Invisalign Express
Best for: Very minor aesthetic corrections only. Typically adults who had orthodontic treatment previously and have experienced slight relapse.
Appropriate cases:
- Minor crowding (1–2 mm) on front teeth
- Small spaces/diastemas
- Minor relapse from previous orthodontic treatment
- Pre-prosthetic alignment (tooth alignment before dental implants or crowns)
Not appropriate for:
- Bite correction of any kind
- Significant crowding or spacing
- Teeth rotations over 20°
- Vertical tooth movement
Refinements: Express does not include refinement rounds. If teeth do not finish as planned, additional trays are an additional cost.
Invisalign Lite
Best for: Mild to moderate crowding or spacing cases without significant bite issues. Common for younger patients or adults with relatively straight teeth and minor alignment concerns.
Appropriate cases:
- Mild crowding (2–4 mm combined)
- Mild spacing between teeth
- Minor arch width discrepancies
- Mild relapse cases that have progressed beyond Express
Not appropriate for:
- Significant bite correction (overbite, underbite, crossbite)
- Extraction cases
- Skeletal issues
Refinements: Lite typically includes 1 round of refinements (additional trays within the same treatment plan if teeth haven’t reached the final position).
Invisalign Moderate
Best for: Patients with moderate orthodontic issues who need more tooth movement but have manageable bite relationships.
Appropriate cases:
- Moderate crowding (4–6 mm combined)
- Moderate spacing
- Class I bite with mild depth/width issues
- Mild overbite correction
- Cases requiring some posterior tooth movement
Refinements: Moderate typically includes 1–2 rounds of refinements.
Invisalign Comprehensive
Best for: The full range of orthodontic problems, including complex bite correction, significant crowding, and multi-arch corrections. This is equivalent to what traditional braces handle.
Appropriate cases:
- Significant crowding or spacing (6+ mm combined)
- Class II or Class III malocclusion (overbite/underbite correction)
- Crossbite correction
- Open bite
- Deep bite correction
- Cases requiring extraction and subsequent space closure
- Complex vertical movements
- Cases requiring rubber band (elastics) use
Refinements: Comprehensive includes unlimited refinements for the first 5 years of the treatment plan — if teeth don’t reach the final position, additional trays are provided at no extra lab cost (though appointment time may incur fees at some practices).
Compliance: The Hidden Cost Driver
All Invisalign tiers require 20–22 hours of daily wear. Non-compliance — removing aligners for meals, drinks, and significantly more than the 2-hour daily window — extends treatment by 30–100% and may require additional refinement trays.
Compliance matters for cost because:
- Extended treatment (extra months) may incur additional appointment fees at some practices
- Non-compliance that requires additional refinements beyond what’s included in the tier may be billed separately
- Your tier was chosen based on a planned movement sequence; skipping wear time disrupts the sequence
Comprehensive vs At-Home Aligners: Which Cases Are Safe for Each
| Case Type | Invisalign Safe | At-Home Aligner Safe |
|---|---|---|
| Minor crowding (1–2 mm) | ✓ | ✓ (with caution) |
| Moderate crowding (3–5 mm) | ✓ | Not recommended |
| Bite correction (overbite/underbite) | ✓ (Comprehensive) | Never |
| Crossbite correction | ✓ (Comprehensive) | Never |
| Extraction cases | ✓ (Comprehensive) | Never |
| Skeletal discrepancies | Requires surgical evaluation | Never |
At-home aligners (Byte, ALIGNERCO, NewSmile) are only appropriate for mild crowding or spacing with no bite issues, and only when a dentist or orthodontist has confirmed you’re a suitable candidate via X-ray review (which some at-home brands do remotely).
Cost by Location (2026)
The same Comprehensive case can vary significantly by region:
| Location | Comprehensive Range |
|---|---|
| National average | $5,500–$8,500 |
| High-cost metro (NYC, SF, LA) | $7,000–$12,000 |
| Major city (non-coastal) | $5,000–$8,000 |
| Suburban/rural | $4,000–$6,500 |
General dentists certified in Invisalign typically charge 10–15% less than orthodontists for the same tier. The trade-off is experience with complex bite corrections — for Comprehensive cases with significant bite issues, an orthodontist is recommended.
