A1C to Average Glucose Conversion Examples
Seeing the ADAG formula applied to real numbers makes it easier to understand what an A1C result actually means in glucose terms. Below are five worked examples spanning the normal, prediabetes, and diabetes ranges, each converted to eAG in mg/dL and mmol/L and to IFCC mmol/mol. The Formulas Used
eAG (mg/dL) = 28.7 × A1C − 46.7
eAG (mmol/L) = eAG (mg/dL) ÷ 18.0182
IFCC (mmol/mol) = (A1C% − 2.15) × 10.929
Example 1 — A1C 5.5% (Normal Range)
- eAG (mg/dL): 28.7 × 5.5 − 46.7 = 111.1 mg/dL
- eAG (mmol/L): 111.1 ÷ 18.0182 = 6.17 mmol/L
- IFCC: (5.5 − 2.15) × 10.929 = 36.6 mmol/mol
This falls comfortably under the 5.7% prediabetes threshold — no indication of diabetes or prediabetes from this result alone.
Example 2 — A1C 6.0% (Prediabetes Range)
- eAG (mg/dL): 28.7 × 6.0 − 46.7 = 125.5 mg/dL
- eAG (mmol/L): 125.5 ÷ 18.0182 = 6.97 mmol/L
- IFCC: (6.0 − 2.15) × 10.929 = 42.1 mmol/mol

Example 3 — A1C 6.5% (Diabetes Threshold)
- eAG (mg/dL): 28.7 × 6.5 − 46.7 = 139.8 mg/dL
- eAG (mmol/L): 139.8 ÷ 18.0182 = 7.76 mmol/L
- IFCC: (6.5 − 2.15) × 10.929 = 47.5 mmol/mol
This is the exact cutoff the ADA uses to diagnose diabetes, typically requiring confirmation with a second test on a separate day.
Example 4 — A1C 7.0% (Common Treatment Target)
- eAG (mg/dL): 28.7 × 7.0 − 46.7 = 154.2 mg/dL
- eAG (mmol/L): 154.2 ÷ 18.0182 = 8.56 mmol/L
- IFCC: (7.0 − 2.15) × 10.929 = 53.0 mmol/mol
An A1C of 7.0% is the ADA’s commonly cited general target for many non-pregnant adults already managing diabetes, though individual targets vary by age, other health conditions, and hypoglycemia risk.
Example 5 — A1C 9.0% (Elevated / Poorly Controlled)
- eAG (mg/dL): 28.7 × 9.0 − 46.7 = 211.6 mg/dL
- eAG (mmol/L): 211.6 ÷ 18.0182 = 11.75 mmol/L
- IFCC: (9.0 − 2.15) × 10.929 = 74.9 mmol/mol
An A1C this high indicates average glucose well above target and typically prompts a treatment plan change and closer follow-up — often quarterly re-testing rather than the twice-a-year schedule used for stable results.
