Mistake 1 — Using the Standard 1.30 Cutoff in Patients 65 and Older

Applying the standard below-1.30 low-risk cutoff to a patient 65 or older, rather than the age-adjusted below-2.00 threshold, produces false-positive “indeterminate” results in otherwise low-risk older patients. Always confirm which cutoff applies before flagging a score as concerning.
Mistake 2 — Treating a Recent Muscle Injury or Hard Workout as Liver-Related AST
AST is present in muscle tissue as well as the liver, and strenuous or unfamiliar exercise, muscle injury, or intramuscular injections can all elevate AST independent of any liver process. Calculating FIB-4 from labs drawn shortly after a hard workout or muscle injury can inflate the score without reflecting real fibrosis risk — checking ALT alongside AST (ALT being more liver-specific) and the clinical context helps catch this.
Mistake 3 — Not Accounting for Statins or Other Medications
Research on statin use and FIB-4/elastography has found that, for a given FIB-4 value, patients on a statin showed different probabilities of elevated liver stiffness compared to patients not on one — meaning medication use is a real confounder worth noting alongside a FIB-4 result, not something to ignore when interpreting a borderline score.
Mistake 4 — Attributing a Low Platelet Count Entirely to Liver Fibrosis
Platelet count can be lowered by many causes unrelated to liver disease — certain medications, autoimmune conditions like ITP, bone marrow disorders, or recent illness. Assuming a low platelet count in the FIB-4 formula reflects splenic sequestration from fibrosis, without considering these other causes, can overstate the calculated fibrosis risk in a patient with an unrelated blood disorder.
Mistake 5 — Treating a Single FIB-4 Score as a Final Answer
FIB-4 is a screening tool, not a diagnosis — a single time-point score, especially in the indeterminate range, is meant to trigger further testing (elastography, specialist evaluation), not stand alone as a conclusion. Mistake 6 — Assuming a High Score Confirms Cirrhosis
