Understanding Your Liver Panel: What AST, ALT & Platelets Mean

Seeing “AST,” “ALT,” and a platelet count on a lab report without context can be confusing. Here’s what each one measures and why they feed into the FIB-4 calculator together.

AST (Aspartate Aminotransferase)

An enzyme found in the liver, but also in muscle, heart, and other tissues — meaning AST elevation isn’t automatically liver-specific. It rises when liver cells are damaged and release their contents into the bloodstream, but it can also rise from muscle injury, strenuous exercise, or other non-liver causes. This is why AST is paired with ALT (below) rather than interpreted alone.

ALT (Alanine Aminotransferase)

A more liver-specific enzyme than AST — it’s found predominantly in liver cells, so an ALT elevation points more directly at liver cell injury specifically. In the FIB-4 formula, ALT sits in the denominator (as its square root), meaning a higher ALT relative to the other values actually lowers the calculated FIB-4 score.

Why AST and ALT Are Used Together

Comparing AST and ALT (sometimes as a ratio) helps distinguish patterns of liver injury — a pattern where AST is notably higher than ALT is associated with alcohol-related liver disease or, in some cases, more advanced fibrosis, while a pattern where ALT is higher than AST is more typical of fatty liver disease and viral hepatitis. Platelet Count

Platelets are small blood cells involved in clotting, produced in bone marrow. As liver fibrosis advances, the spleen often enlarges and starts trapping and holding onto more platelets (a process called splenic sequestration), which is why a falling platelet count over time can be an early, indirect signal of worsening liver fibrosis — even before AST or ALT show dramatic changes. In the FIB-4 formula, a lower platelet count increases the calculated score, reflecting this relationship.

Age’s Role in the Formula

Age is a direct multiplier in the FIB-4 formula, reflecting that a given AST/ALT/platelet pattern carries more fibrosis significance at some ages than others, on average, across the populations FIB-4 was validated in.
